AV Equipment Reservation Form

Video Class Reservation Form – Available only to students currently enrolled in a Haverford Video production course

Loan Policy Overview

  • AV equipment is available for loan to Haverford faculty, staff and students only.
  • Reservations must be made at least one business day in advance. Please reserve your equipment at your earliest convenience as last minute requests cannot be honored.
  • All reservations must be submitted using the form below and cannot be submitted by phone, walk-in, or email.  All reservations will be confirmed via email.
  • Equipment must be picked up and dropped off between the hours of 10:00am and 4:30pm on weekdays.
  • Our standard loan period for all equipment is 2 days.  Extensions are possible, at the discretion of AV staff.
  • Equipment is available on a first come-first served basis.
  • There is no charge to borrow equipment.
  • Training: You should know in advance how to operate the equipment that is being requested. Our staff may not be available to show you how to use the equipment when you come pick it up or to assist you during your event. However, you are more than welcome to make an appointment with a member of our staff to learn how to operate the equipment prior to using it.

Responsibility for Equipment

  • AV equipment is your responsibility while it is in your custody.
  • By submitting the reservation form you indicate your acceptance and responsibility to replace or repair lost or damaged equipment. Loss or damage of equipment while on loan is the responsibility of the individual/department to which it is on loan and replacement or repair costs of equipment will be charged to those individuals/departments who fail to return technology resources in the same state in which they were loaned.
  • Please do not leave equipment unattended and use every precaution to ensure the safety of technology equipment.

Obtaining Equipment

  1. Complete and submit the reservation form (below.)
  2. Wait for confirmation of your reservation.
  3. Pick up and return the equipment at the ITC (Instructional Technology Center) located in Stokes 205 at the time indicated in your reservation request.
